Biography
Certina Ho, R.Ph., B.Sc.Phm., M.I.St., M.Ed.
A graduate of the Faculty of Pharmacy, University of Toronto, Certina Ho is currently is a Project Manager at the Institute for Safe Medication Practices Canada (ISMP Canada) with a primary responsibility on medication safety initiatives related to community pharmacy practice as well as medication incident reporting and analysis. Certina obtained her graduate degrees in Library and Information Science in 1998 and in Education in 2005.
She is an Adjunct Clinical Assistant Professor at the School of Medicine, McMaster University, providing teaching support to evidence-based medicine tutorials and quality assurance projects of family medicine residents at the Kitchener-Waterloo campus.
In addition, she has been an Adjunct Clinical Assistant Professor at the School of Pharmacy, University of Waterloo, since 2008 with a focus on the development of drug information, evidence-based medicine, and patient safety curriculum.
Currently, she is a faculty coordinator at the Office of Experiential Education, Leslie Dan Faculty of Pharmacy, University of Toronto, responsible for the Advanced Pharmacy Practice Experience (APPE) preceptor and site recruitment.
